Course Overview

The SCALE™ Serious Incident Analysis: Foundations online course has been designed for safety professionals, including advisors and investigators who play a key role in managing workplace incidents and improving safety outcomes. Across six modules, articipants will develop the knowledge and skills required to:

• Collect critical information from workplace incidents
• Decide which incidents are of high severity potential.
• Under the importance of controls and enablement
• Analyse serious incidents to identify and prioritise causal factors
• Develop and recommend effective corrective actions that reduce exposure
• Analyse and interpret safety data to support operational decision-making

Through evidence-based research, practical applications and real-world scenarios, this course equips professionals with the essential tools to enhance workplace safety and incident management.

Learning Outcomes

Upon completion, participants will:

• Be able to assess incidents for their potential to cause serious negative outcomes, supporting informed decisions on whether an investigation is required and the appropriate type of investigation.
• Gain an in-depth understanding of how to analyse control failures that contributed to the incident and evaluate whether the organisation has effectively enabled safe work practices.
• Develop the capability to conduct thorough causal analyses, including examining cognitive aspects of human error, the influence of operational culture and management practices, and the impact of misaligned organisational systems.
• Learn to prioritise investigation findings effectively, identifying the most critical causal factors that require resolution.
• Acquire the skills to develop precise and impactful corrective actions that meaningfully reduce risk and improve workplace safety.
