Presentation: GN Residential Construction Pty Ltd and Synergy Scaffolding Services Pty Ltd.
This presentation is a case study on a high profile scaffold collapse which killed 1 person, seriously injured another and put at least 2 workers at risk of serious injury or death. The discussion will include the complexities of this investigation, the challenges faced, the lessons learnt and how the significant outcome made history not only within industry but also within the NSW work health and safety jurisdiction.

Presenter: Leonie Ball, State Inspector with SafeWork NSW’s Investigations & Emergency Response Team.
Leonie joined WorkCover in 2008 after working as an Electrician with Sydney Trains for 15 years. Leonie completed 2 years in the construction team before moving into the investigation’s unit. Leonie has over 14 years’ experience in conducting investigations and responding to serious incidents and fatalities. Leonie is also involved in training new inspectors in first response and investigations activities.
